Adoption Application

Thank you for your interest in adopting one of our animals. Please be advised that all the information you provide must be as accurate as possible.

If you are interested in adopting multiple animals, you only need to submit one application and put all the names of all animals of interest in the application.

**Our puppy adoption donation covers the cost of rescuing canines from high kills shelters, including transportation, medical costs, feeding, supplies, etc. which are required to take care of our rescued animals. We are a registered 501(c)3 organization. All puppies are up to date, for their age, with their booster shots, have bordatella vaccinations, have been de-wormed, and have been vetted prior to being adopted. All dogs are micro-chipped and registered to you at the time of adoption.

It is CARE's policy to not adopt to households with children younger than 6 years old.

While we understand we may be passing on an excellent home, we feel it is in the best interest of our pups.

We cannot accept applications from individuals under 21 years of age. Also, if the applicant is living with someone that owns the home, we require that the homeowner (deed holder) completes the application.
